Rogers, Clerk of the Board Attachment A Petition for Formation of Mason County Pub- lic Hospital District No. 2, Mason County, Washington BEFORE r HE MASON COUNTY BOARD OF COMMISSIONERS MASON COUNTY, WASHINGTON In the Matter of Mason County Public Hospital District No. 2, Mason County, Wash- ington We, the undersigned qualified voters who are residents of the territory of a proposed public hospital district, do hereby respectfully petition the Mason County Board of Commis- sioners of Mason County, Washington, and represent as follows: I. There is a present need to form a public hospital district to be known as Mason County Public Hospital District No. 2. Mason County, Washington (the "District"), and the proposed territory hereinafter described should be formed into such a district to furnish health care facilities and services to adequately pro- vide for the residents of such area and others as soon as reasonably possible. I1. The boundaries of the proposed Dis- trict should encompass all lands lying within the currently existing boundaries of the voting precincts of Belfair #1, #2, #3, #4, #5, and #6, Tahuya, and Dewatto. The boundaries of the proposed District follow existing pre- cinct boundaries and do not divide any voting precinct, all as required by RGW 70.44.030. Prior to the formation of the proposed Dis- trict, the territory to be formed into the Dis- trict would be withdrawn from Mason County Public Hospital District No. 1. Ill. The proposed District should be estab- lished with a Board of Commissioners com- posed of three commissioners all of.whom should be elected as at-large positions as authorized by FICW 70.44.040. IV. Pursuant to the laws of the State of Washington. after notice, hearing and con- sideration by the Mason County Board of Commissioners, a proposition for the forma- tion of the District should be submitted to the voters within the territory of the proposed Dis- trict for their approval or rejection at the next available general or special election as de- termined pursuant to chapter 29A.04 RCW. At such election, three commissioners for the District should be elected pursuant to the pro- cedures specified in RCW 70.44.040. WHEREFORE, your undersigned peti- tioners ask that the Mason County Board of Commissioners: 1. Fix a date, time and place for a hearing on this petition, and that published notice be given of such hearing in the manner provided by law; 2. Upon such hearing being had, make and enter an order approving the petition, and establishing and defining the boundaries of the proposed District; 3. Adopt a resolution submitting the ques- tion of whether or not to form the District to the voters within the territory of the pro- posed District at the next available general or special election as determined pursuant to chapter 29A.04 RCW, and further provid- ing for the election of public hospital district commissioners; and transmit such resolution containing a proposition for the formation of the DistriCt and providing for the election of commissioners therefor to the Auditor of Ma- son County, Washington, so that the Auditor may proceed with such election in the man- ner provided by law; and 4.
